This is the base fee for a 15-minute consultation to discuss and evaluate 1-2 problems.  Problems include acute illness/injury, smoking cessation, medication evaluation and management, specialist referrals and much more.

Simple exam you can attend from anywhere.  From sick notes to med refills, many times there is no need to actually come to the office.  When you need to be seen or have a few questions or need some health advice, this is a visit you can do from home.  See the Telehealth Page on this site to find out how to contact us for a Telehealth Vist.

This visit includes the physical necessary to remain licensed as a commercial driver.  It includes urinalysis, but not drug testing.  The Urine Drug Screen is a separate fee.

This is a visit that lasts longer than 15 minutes and we discuss, evaluate and treat 3 or more problems.Problems may include chronic health conditions, such as hypertension or diabetes, evaluation of wounds, skin problems or more.

This would be a follow-up appointment after an illness or a consultation for new moms who need some advice or assistance.

This visit includes the physical necessary to have your student included in extracurricular activities/school sports.  Please bring the school's required forms with you to the appointment.

At this time, all blood work and other laboratory testing is sent out by courier to a contracted laboratory.  We usually receive same-day results, however, please allow 24 hours for all send-out testing to be resulted.

The recommended annual Well Adult Physical includes routine check of vital signs, weight and evaluation of overall health.  Labs may be recommended during your physical, which are separate fees and billable to insurance.  

This visit includes women's reproductive health screens, such as a pap smear with pelvic exam. Consultation for family planning is included and reproductive concerns are addressed and treated.

This physical is necessary for resident immigrants to qualify for citizenship.  It includes the basic physical and basic required labs.  At times, we may need to run additional laboratory tests.  The fee for additional testing starts at $50 and goes up dependent on what is needed.
